Both the health history form and insurance form must  be
printed (either hit Ctrl-P, or click file->print on your toolbar),
completed, and returned to the office of Obstetrical Anesthesia
Associates. It is necessary that these forms be returned to us
so that we can be assured that you are informed about
epidural anesthesia. You may return these forms by printing
them out and mailing them back, faxing them in, or emailing them in.
